USAV and their insurance company, effective immediately, is requiring all regions to use their new USA Volleyball 2011-2012 Individual Membership Form. This form takes the place of our old Waiver and Release of Liability form. The form can be found at www.ovr.org/forms/USAV_Membership_Form.pdf. Please print out this form and use it effective immediately for your tryouts and all future 2011-2012 registrations.

This is a two-page form which requires the member to legibly print their personal information at the top including the Juniors Only box and then requires three signatures to complete the form. Forms that do not have the three signatures will not be accepted per USAV and the insurance company.

Please note that the OVR will accept the old Waiver and Release forms that were signed early or forms that have already been sent to the OVR or collected by the club director. The new USA Volleyball 2011-2012 Individual Membership Form should be signed by all new members completing their paperwork after the date of this e-mail.

Please note that the OVR fees listed on the OVR website are the only fees that members have to pay to complete their registrations.

Please leave the membership options section blank on the new USA Volleyball 2011-2012 Individual Membership Form. The $5.00 listed on the new form is an optional fee, in addition to the regular fee, to make a donation to USA Team Programs. The $6.85 fee is for officials to purchase extended officials' insurance, which is in addition to their registration and background screening fees.
